Uncontrollable Facial Twitching? Understanding Hemifacial Spasm

Get to know the causes, symptoms, and treatment options for hemifacial spasm to improve quality of life.

Hemifacial spasm is a disorder of the facial nerve that causes involuntary, repetitive twitching or muscle contractions on one side of the face. This condition most commonly occurs due to pressure from a blood vessel on the facial nerve.

According to dr Wienorman Gunawan, Sp.BS, Neurosurgeon at Bethsaida Hospital Gading Serpong, hemifacial spasm develops as a result of disruption to the facial nerve caused by compression from surrounding blood vessels.

“As we age, blood vessels tend to lengthen and lose elasticity, which may lead to pressure on the facial nerve. This is what causes abnormal facial muscle contractions,” he explained.

This condition not only causes physical discomfort but can also affect a patient’s self-confidence, particularly among women. Persistent twitching may interfere with facial expressions and cause embarrassment during social interactions.

Treatment Options for Hemifacial Spasm

For patients whose symptoms do not improve with oral medication, two main treatment options are available:

Microvascular Decompression (MVD)

This surgical procedure aims to relieve pressure from the blood vessel on the facial nerve by separating the nerve from the compressing vessel.

Botulinum Toxin (Botox) Injections

This method involves injecting botulinum toxin into the affected area to reduce excessive muscle contractions. The treatment is typically administered periodically, depending on the patient’s condition and response.

“Both approaches have their respective advantages and should be tailored to the patient’s specific condition. Therefore, consultation with a neurosurgeon is essential to determine the most appropriate treatment,” added dr Wienorman.

Bethsaida Hospital Gading Serpong offers a dedicated Neuroscience Centre equipped with comprehensive facilities to manage a range of neurological disorders, including hemifacial spasm. Supported by advanced medical technology and an experienced team of specialists, patients can receive personalised care tailored to their medical needs.

Dr Pitono, Medical Director of Bethsaida Hospital Gading Serpong, emphasised that the hospital’s Neuroscience Centre provides a full spectrum of services, from diagnostic assessments and invasive procedures to minimally invasive and modern therapeutic treatments.

“We developed this service with empathy, recognising that facial twitching can persist for an extended period or even a lifetime. Appropriate management of facial twitching can significantly improve patients’ quality of life and comfort in daily activities,” he stated.
